Going to a legitimate gender therapist for a letter is NOT difficult, and need not entail any long stretch of time. Depending on your level of preparation and decision certainty, it can be as little as one visit for an evaluation, with a follow-up visit just to verify that the letter is an accurate reflection of your situation/needs. Those who desperately fear going through this minor "hoop" frankly need to grow a pair, before they can get a pair chopped out!

So if someone's interested in castration for libido reduction rather than sex reassignment, what sort of therapist would they seek out? How achievable would getting a letter be in that circumstance?

I do not have personal experience in the area which you find yourself. My guess is that you'd not get a letter to be physically castrated for libido reduction. However, they might work with you on a plan to help get you to chemical castration which could help that. In your case, I think I'd go the route of trying to get help with being chemically castrated first. Then you could later go to Dr. Arnkoff to have a no-letter castration to make it permanent, if you find the chemical castration satisfactory.
You're a poster child for try before you buy, because you want libido reduction. In my case it wouldn't have worked because my goal was not libido reduction, it was being uncomfortable in my body with testicles attached.

You can get therapists that will support castration for sex addiction. I've gotten several over the years to do chemical castration prescriptions. You basically have to just tell a compelling story of how your compulsive sexual interests are distressing your life. For me I usually told them I was addicted to prostitutes and porn (which I was) and it was risking my marriage, work and health. Some doctors even specialize in this such as Dr. Renee Sorrentino in Massachusetts. If you take the chemical castration for two years and progress that it is working week they will support surgical castration. Castration, at least chemically, is quite commonly prescribed for any paraphilia that is causing distress or harm.

I am in the process of castration evaluation with a GP. If I end up with chemical or physical castration or even not having either but getting my hypersexualiy under control I will be glad I got the real deal. A few hundred dollars is nothing compared to a bad decision. I think many of think about castration but are not willing to come out to the ones who may put us on a different path. My fantasy is becoming a reality of my life. My hyper sexuality is real, just not sure about the castration. As much as I love the look of a neutered man and want it myself I am going to make sure. Dr. Arnkoff is the only one I know of. I had orchie and penectomy with him. He's amazing I would be so miserable if he did not help me.

Dr. Mark Meadows in Chicago, IL, did mine without a letter. Just took my word for it that I'd talked about it with a therapist. Fast, too.

I am a patient at that practice. I had a female urologist, who was heavily pregnant at my last appointment, and . didn't come back after her maternity leave. She performed a prostate biopsy on me. After some very high PSA results and a couple of other things, they were pretty confident, 95% sure, my biopsy would be positive . We discussed what options i would face. Surgical castration was not one she talked about. I brought it up, and she was very much against it. I kind of expected a female urologist to not be so squeamish about it. The biopsy turned out, much to everyone's surprise, negative!

My urological surgeon just required proof that I had gender identity disorder diagnosed and treated with HRT for at least 6 months. I wish I had found her earlier.
