The position I am in is very probably not the same as yours, but unpicking it may help.

I have had testicular torsions since the age of 9, so I went through puberty with this awareness of the threat of intolerable pain hanging over me at any moment. I have also had a castration fetish for as long as I can remember.

It took me years to come to this conclusion, but for me it is all about the fantasy. Sure, I would love to have my dick cut off, but then the fantasy ends.
