Not sure of permanent damage, but lack of regular erections does lead to shrinkage. It may be permanent, if it is long term.

Never used one, or wanted to, and now no point in trying and I don't think I could, so opinion only...

I suspect how using a chastity device works for a person is probably going to be a question of what their expectations / reasons / attitudes about it are, and possibly the sort of device design.

I hadn't seen any real discussion of injuries from using them, other than the occasional tale (usually with rude remarks) about some guy getting a device (usually a cock-ring) stuck and having to go to the ER and get it removed. However just out of curiosity I decided to do a StartPage search on "injury from use of chastity cages" I found a good bit ranging from AI summaries, to forums on chastity, to articles posted by places selling them.

Essentially most said there is not a lot of risk from a properly fitting and made device, used with proper care for cleaning, and monitoring for any signs of problems. Most of the problems amount to sores and other skin issues. However if a device is to tight or otherwise ill fitting, and not watched for signs of issues, you can get damage from loss of proper blood flow.... All of the sites that advised on 'how to' emphasized the need for proper sizing and extreme attention to hygiene...

All of this seemed pretty intuitive, and was about what I expected just from thinking about what wearing just about any sort of snug fitting item on any part of the body.

It's real that getting accustomed to a chastity device could be an ordeal. The size, the quality, the material, the conception so much parameters to make it possibly comfortable or the opposite. All depending on your motivation and anatomy. I could say that it took me more than a year to really get comfortable with some of the one I had purchased. But it's now, and in the long run, that I got this nice dickless feeling with the small cages. Those cages, are in my opinion, the one's that are compatible with the search of a pseudo castration experience. I agree that there is a lot of kink in the use of chastity devices but again, in my observations on dedicated web sites, the kink is mostly searched by sexually active guys (gay or strait) and the type of cage used could then also be different. They would use sizes that give a little more room compared to the flat cages and with some space between the bars that gives more contact with the clothing, maintaining some arousal. When using this type of cage, with a younger age, with a stronger sexual drive, the kinks are more than welcome. It becomes a complex sexual play.
About the damage to the penis it could also depends on many variables. The longer the worse of course but with a smoother approach it could be with no effect for years. But it largely depend on the individuals and their age. For myself near the seventies, the sexual drive is much lower compare to my fifties and I have the motivation, with the help of the device to engage in a more deeper thoughts for castration. If longer encaged period brings me dysfunctional erection it would be more than welcome. I would feel it as reason to get castrated for good.

You made very good points Wheely. Especially in the above quote. My reasons, expectations are very of my own and may not fit the ones of another guy much younger in a sexual active relation. What I feel as a pseudo castration using a small or flat cage is accordance with what I live and feel in my life right now. It will evolve in the near future and it would be my journey with my feelings and my decision. For now I'm happy with that dickless feeling as I woke up every morning with a smile on my face. A kind of pleasure to put away the sex drive. How it really works I don't know, but by not seeing, feeling or touching the penis it like it's gone away in a certain manner. Needless to say that the older age is quite helping I guess.
